Looking for a therapist in Australia

Hi there I'm new to this site, my question goes out to anyone who knows if there are any therapists in Australia -Queensland - Brisbane/gold coast area, can't find anyone who is a pelvic specialist, I had trauma to my pelvis 2 years ago while working as a volunteer in a rescue organisation, I have been diagnosed with Pudendal nerve entrapment, probably more injuries, I can't find any one here in Australia, we are told to just deal with it, I am on morphine now, and just looking for HELP :62_58_40:

Who diagnosed you with pudendal nerve entrapment, and on the basis of which tests? Also, please exactly describe your injury. Thanks.

Thank your for your reply, it was a neurologist, I had the symptoms of pudendal nerve entrapment, I also had 3 nerve blocks I did have some success especially the first one the other 2 were done by a different doctor, but all 3 nerve blocks were done at the ischial spine none were done at Alcock' canal, there was no specific timing in the protocol to the steroid injections.

Now my injury occured in September 2006 while working as volunteer on a prototype rescue vessel, I sustained several high impacts to my pelvis in heavy seas, as I was forced to sit down, due to the design of the vessel ' it is a very bad design

I am trying to find a EMG test at the moment to see if this will show any damage ??? But I can't find anyone to do a pelvic exam on me to establish what damage I have. As I said I asking does anyone know of anybody who is skilled, at the moment I am in a living hell when it comes to pain, and I am in a very dark place mentally :boese:

Can you please exactly describe your symptoms (every detail please)? To what extent are your symptoms worsened with sitting?

I'll give you my symptoms in detail as best as I can, I just done this, but my PC crashed so I'll try again, My pain is worsened by sitting, my pains are; I have pain in the testicle it was mainly the right one but it it is both now, but still the right one is being predominant; I also get pain in the right groin; I had physiotherapist vigorously jam his elbow into my upper and lower buttocks and outside thigh; which gave me extra symptoms and more pain; I also have pain in the tip and head of the penis, it can only be described as intense, even the sensation can change, when it is intense it will drive you to insanity; I also have pain in the sacrum, mainly when I sit on my donut cushion when I drive, it fells like my sacrum has seized up when I get out of the car [the drive is only short, anyway ] I also have pain in the S.I. j. now, which radiates to my lower back; [What has gave me the best relief is >>> I spend most of my time on my back with my feet on the chair and a heat pad under my pelvis and lower back, laying there doing nothing, so I don't aggravate my injury or pelvis; crossing my legs will bring on groin pain, so I try to keep the bio mechanics in check; I also have pain in the anus sometimes, and some leakage sometimes; I also have urinary problems sometimes, ; erectile problems; and I had pain after ejaculation I think that's all, but that's most of it; and as I said I can't find anyone to properly help me, which I am getting progressively worse because of inaction and I have been given morphine, Valium, neurontin, panadeine forte and antidepressants to deal with my problem :62_58_40:

oh yeah there is pain in both buttocks and perineum , :confused:

There is one question is there numbness associated with PNE as my insurance company reduced me from 10 % to 5 % impairment ,because i didn't have enough numbness in my penis after they jabbed a toothpick into my genitals :refilao: [they call this a pin prick test ] but it was a toothpick jab test ' so is numbness a PNE criteria :shock:

Hi Ausie,

I will leave Webslave to remark on your specific situation and symptoms, as he is much wiser than I. However, you should know that there's a great deal of debate on the existence of PNE. Even many of those who do believe in it denounce the surgery, and I would urge you to be just as skeptical. My doctor, the head of pain management at University of Miami (who has invented methods of pudendal nerve block) says he would absolutely never operate on/transpose the nerve.

Regarding the test you had ... Sensory loss is a suggestive symptom of nerve damage or entrapment. Some doctors believe they can diagnose PNE partly by way of a pinprick test, heat sensory testing, etc.

I do wonder if you've ever had an MRI. Your symptoms are not out of the area of pelvic floor dysfunction, but they could also be widespread enough to maybe be a nerve root impingement? Sorry, I'm no doctor. :)

Age: 26 Onset Age: 17 Symptoms: Shooting, nerve-like pains throughout the penis, which abruptly hit and leave. Testicular pain, perineum pain, burning/irritative urination, extended pain after ejaculation. Occasionally, some allodynia or ache in the coccyx/sacrum/thigh/buttocks/legs. Diagnosis: Pelvic floor dysfunction, degenerated lumbar disk, and mildly herniated lumbar disk. Helped By: Physical therapy, pain management doctor, hot baths, therapy pool, stretching regimen, breathing exercises, relaxation, distraction. Worsened By: Arousal/ejaculation (worst), constipation, panicking/obsessing, other triggers depend upon current symptoms. Tests/Prior Treatments: Too many antibiotics to count, multiple urine tests (all normal), testicular ultrasound (normal), bladder and renal ultrasound (normal), lumbar and pelvic MRI with and w/o contrast (revealed disk problems), Elavil 25mg (caused retention), Flomax 0.4mg.

Ausie, unless your symptoms specifically occur or worsen when you SIT, then this is not PNE. In addition, you do not have the numbness required for a PNE diagnosis. Your symptoms are in fact typical of people with CPPS, especially the tell-tale post-ejaculatory symptoms. This is good news for you. chronic prostatitis / chronic pelvic pain syndrome is controllable and often largely curable; PNE (when it rarely exists) can require serious surgery that is only successful in about half the cases.

So throw away the PNE diagnosis and never see the doctor who gave you that diagnosis again.

As to what to do — this website is full of good advice. You will get better. :smile:

thank you for your advice, just to clear up the sitting factor, yes my symptoms do get worse when I sit, so you are saying I don't have PNE, as I thought that when I get my symptoms when I sit, I had PNE, other wise I'm getting hope

thank you for your response, yes I had a MRI it did not show anything, my next test is a EMG so my pain specialist can do a nerve block more accurately, will a EMG show anything ?

People misinterpret the sitting test for PNE. You have to go from asymptomatic (no symptoms) to sudden, clear symptoms when you go from standing to sitting in order to call this PNE. Many men think they have PNE because after a lot of sitting their perineal pain seems worse, which is a completely different issue.

yes this was the original condition when I sat down, the pain in the left testicle came on and the lower back and groin, but now I am in pain all the time, but it gets worse when I sit, this pain came on a couple days after trauma , I got pounded while sitting in a prototype rescue boat on a Sunday, then on a bus trip on Tuesday the pain had hit me. Before that I was pain free

Well, if your pain clearly only came on when sitting and immediately followed significant trauma to your pudendal nerve, I suggest you go to the pudendal nerve entrapment forums (google "tipna"). Beware of these forums though — they are merely shopfronts for surgeons to sell their services, in my opinion.

In your shoes I'd first travel to see an expert like Tim Sawyer in California before embarking on anything related to PNE though. If he finds trigger points that re-create your symptoms, you can discard the PNE diagnosis.

so you think I might have PNE, so who is Tim Sawyer, and how do I get in contact with him, Now will a EMG show if I have PNE, I wonder if trigger points will give me some relief, I am reading A Headache in the Pelvis at the moment and I see all my symptoms, I had hope ; I guess trauma is the cause of PNE, I don't feel as happy now, after you said I didn't have PNE, I am looking fore answers to my problem, do you suggest anything that might reveal how bad the PNE is, and yes I am on the pudendal web site looking for answers, hoping physical therapy could be a solution . I live in Australia and no DOCTORS treat men with these problems. :sad:

now one more question about the numbness of the penis, I was reading the article on PNE it said trauma and numbness of the penis, is numbness a criteria of PNE ? how much numbness do I need, as it would depend on the amount of compression you have. I am learning as much as possible . I want to know if physical therapy will help ? If I have PNE I want to be 100 % sure
