I don't completely agree with the content of this video, and I have always felt that phenotyping carries the risk of classifying patients incorrectly. For instance, Payne says that "ulcerative IC" is a distinct disease, but there is some evidence to suggest that common BPS can, via neural cross-talk, up-regulate the nerves in adjacent structures, causing mast cell proliferation and degranulation, and leading to ulcers. So in some people, the myofascial phenotype could conceivably lead to what looks like a bladder disease. The same potential overlaps and confusions could be said of the other so-called phenotypes.
I still strongly suspect that the move to phenotyping is an attempt by urologists to tame this mysterious malady. I have yet to be convinced that it is correct.
