Internal hemorrhoids are around the sphincter and will not affect internal massage. You should have the internal hemorrhoids assessed by a surgeon with a view to fixing.

See if symptoms change (IOW improve) when you're in vacation mode. That's a 'tell' if it occurs, meaning you are in control of it and must devote more time to instilling calmness into your life. Easier said than done.

If you are not experiencing PAIN, then you do not have CPPS, yet, and you may never have it. You may need treatment for OAB. Have you been evaluated for OAB? Holding urination until you are uncomfortable is not good. It acts to heighten the neural windup that needs to be subdued.

Could be simple flare, which can happen to anyone (difficult to tease out the triggers, but often stress or worry, change in diet, change in activity etc), or could be a reaction to kratom, which is a largely unknown and potentially dangerous drug:

I have permanent tinnitus in one ear. It was driving me mad until I simply decided to ignore it. As soon as I did that, it stopped bothering me and now I have to concentrate to hear it.

Google 'cymbalta and tinnitus' and 'tizanidine and tinnitus.'

FYI tinnitus is classed as an uncommon side effect of cymbalta (0.1% to 1% of patients), and not associated with tizanidine.

Tinnitus is a consequence of quite a few antibiotics, if you've had a lot of them...
